Our one and only mission at Kare Center is to encourage, as well as facilitate healthy behaviors for a healthy lifestyle. It is already a big challenge in our minds to be diagnosed with a chronic disease, but having the zeal and motivation to choose a healthy lifestyle can even be more challenging.
At the Kare Center Disease Management Clinic, our patient-centric approach to disease management makes it possible for you to actively participate in the whole process of giving you back the quality of life that you desire and truly deserve.
It doesn’t matter the level or progress of your medical condition, we can always start with you at any point. We will mutually create your health goals, and we will see to it that these goals are met.
We Manage the Disease; We Engage Our Patient
One important aspect of managing diseases is taking cognizance of the patients themselves. Any treatment commences with the individual and their ability to heal from within. Hence, we make sure to provide our patients with the right amount of mental and emotional support and motivation that they need.
In the same vein, we are quite aware that chronic diseases like infectious diseases, complicated diabetic wounds that won’t heal, and diabetes itself come with emotional trauma, and that is the real challenge.
Our disease management, in the form of therapy, begins with an awareness of what makes each chronic condition and, more significantly, each individual’s unique demands and requirements in order to help them accomplish meaningful and permanent lifestyle changes for better health.
We are a compassionate group of professionals that work from the abundance of our passion for our jobs. The enhanced health and well-being of our patients is our greatest reward. Each patient is cared for by a multidisciplinary team of doctors. In their own paths to improved health, they listen, inspire, advocate, educate, and sometimes laugh and grieve together.
Our Disease Management Clinic Mission Statement:
To address the health care requirements of our patients by providing evidence-based practice and education through a team of physicians, registered nurses, and registered dietitians, with an emphasis on diabetes, diabetic wounds, infectious illnesses, and vascular diseases. We advocate for preventative healthcare for patients, families, and caregivers in order to improve the quality of their lives now and later.
